Introduction
Breast cancer survivors face a variety of physical, psychological, and social needs. Despite this, comprehensive post-cancer care that addresses all patient needs is often inadequately provided.
To enhance patient autonomy and empowerment, and to maximize the uptake of multidisciplinary supportive care, we implemented a proactive post-cancer care pathway for patients with early-stage breast cancer upon completion of the active treatment phase.
This study aims to describe the usage, adoption, and perceived utility of the proposed patient pathway, as well as the main challenges encountered during its pilot implementation phase.
